Directions to Cumberland site

Our Cumberland facility is located at 525 Broad Street, in the lower level of the William E. Blackstone Medical Building, across from Building 19 (formerly Ann & Hope).

To Cumberland site From Providence

Take Interstate 95 north to the Lonsdale Avenue (exit 26) in Pawtucket. At the lights, turn left onto Lonsdale Avenue North (Route 122). Go about 3 miles, turning right at the sixth set of lights (Ann & Hope Way). Proceed about 1/3 mile to the lights at the end, which is the intersection with Broad Street. Make a right and then an immediate left into the Blackstone Medical Building entrance.

To Cumberland site From Woonsocket and Worcester

Take Route 146 South to Interstate 295 North. Take exit 10 for Mendon Road (Route 122) South. Proceed about 2.5 miles, and turn left at the traffic lights onto Broad Street. The Blackstone Medical Building is about 1/2 mile on the left side, at a set of traffic lights.

To Cumberland site From Attleboro/Boston

Take Interstate 95 South to exit 3B for Route 123 West. Take 123 West for about 5 miles (past Routes 1A and 1 to the end, which will be Broad Street). Turn right onto Broad Street, and the Blackstone Medical Building will be about 1/4 mile on the right side.

Directions to East Providence site

Our East Providence facility is located at 1002 Waterman Avenue near Interstate 195 and Route 6, close to Seekonk, Massachusetts.

From Interstate 195, Providence

Travel east on Interstate 195. Take RI exit number 8 and turn left at the lights at the end of the ramp (Warren Avenue). Turn left at the third set of lights (Warren Avenue), and then left at the first set of lights which is Waterman Avenue. Our building is two blocks ahead on the right side, at the corner. Parking is available directly in front of the building or beside it on Humphrey Street.

From Interstate 195, Massachusetts

Travel west on Interstate 195. Take MA exit number 1 and follow the ramp to the right, onto Route 114A North (Fall River Avenue). Follow this about one half mile and turn left at the lights onto County Street. Turn right at the first set of lights which is Waterman Avenue. Our building is two blocks ahead on the right side, at the corner. Parking is available directly in front of the building or beside it on Humphrey Street.

Directions to Warwick Site

Our Warwick facility is located at 335 Centerville Road (at the northeast corner of Route 117 at Interstate 95) in Building 2 of the Office Commons 95 office park.

To Warwick Site From Providence

Take Interstate 95 or 295 south to exit 10A of Interstate 95. Take Centerville Road eastward (Route 117), proceeding under the bridge and changing to the left lane. Make a left at the first opening in the median, which turns you westbound, back towards the bridge. Just after turning, take the first right (just before the Office Commons 95 sign) and proceed to Building 2 in the front row of single story brick offices.

To Warwick Site From South County and Vicinity

Take Interstate 95 or Route 4 north to exit 10 of Interstate 95. Stay left at the end of the ramp before stopping. Cross the eastbound lanes to the median, and then take a left onto Centerville Road (Route 117) west towards the bridge. Just after turning, take the first right (just before the Office Commons 95 sign) and proceed to Building 2 in the front row of single-story brick offices.

Directions to Westerly Site

Our Westerly facility is located at 101 Airport Rd., just south of the airport in the Westerly Professional Center.

To Westerly site from Interstate 95, Rhode Island

Take RI exit 1 from Interstate 95 onto Route 3 south (Main St.), and travel about 4.5 miles. Just after the overpass, take a left at the Route 78 East ramp. At the first traffic lights (about 2 miles), Route 78 crosses Route 1 and becomes Airport Rd. Proceed straight for another mile, turning right at the Mastuxet Professional Park sign, to the Westerly Professional Center building on the right side of the driveway.

To Westerly site from Interstate 95, Connecticut

Take CT exit 92, for Route 49 to N. Stonington/Pawcatuck. Turn right at Liberty St. (Route 2), proceed about 1.2 miles and take route 78 East, toward Westerly/Misquamicut beaches. After about 4 miles, Route 78 crosses Route 1 and becomes Airport Rd. Proceed another mile, turning right at the Mastuxet Professional Park sign, to the Westerly Professional Center building on the right side of the driveway.

Directions to Providence Site

Our Providence facility is located in the West River Center at 148 West River Street, which is the street behind the main Providence Post Office.

From Route 95, Northbound

Travel North on Interstate 95 onto the Route 146 North ramp, and immediately take the exit for State Offices - Route 44. Turn left at the end of ramp which is Orms Street, and left onto Charles Street. Go about .4 miles to West River Street and turn right. The building is .4 miles ahead on the left side. Use the West River Center East entrance.

From Route 95, Southbound

Travel South on Interstate 95 and take exit 24 for Branch Ave. Turn right at the end of the ramp, then take a left at the lights onto West River Street. The building is about .3 mles ahead on the right side. Use the West River Center East entrance.

From Route 146, Southbound

Travel South on Route 146 to the Branch Ave exit. Turn left at the end of the ramp. Travel about .9 miles and then take a right onto West River Street. The building is about .3 mles ahead on the right side. Use the West River Center East entrance.

Directions to North Smithfield Site

Our North Smithfield facility is located at 501 Great Road (Rte. 146A) in the Branch Village Professional Offices about 1/2 mile east of Route 146.

To North Smithfield site from Rte. 146, Rhode Island

Travel North on Route 146 and take the Route 5/102- Forestdale/Slatersville exit. Turn right at the stop sign, and right at the lights onto 146A/Great Road. The Branch Village entrance drive is .6 miles on the left. Proceed up the entrance drive, turning left at the top. Our facility is located towards the end of the building in suite 202.

To North Smithfield site from Rte. 146, Massachusetts

Travel South on Route 146 and take the Route 5/102- Forestdale/Slatersville exit. Take a left at the end of the ramp onto 146A/Great Road. The Branch Village entrance drive is .7 miles on the left. Proceed up the entrance drive, turning left at the top. Our facility is located towards the end of the building in suite 202.
